During a stay in hospital your medicines may be changed. Studies show that many patients may experience an error or problem with their medicines after they have been discharged. This service supports patients recently discharged ensuring that changes to their medicines made in one care setting (e.g. during a hospital admission) are enacted as intended in the community helping to reduce the risk of preventable medicines related problems and supporting adherence with newly prescribed medication.
